Introduction

At the Heart of Kent Hospice, we offer specialist hospice (palliative) care for people and their families affected by a life-limiting illness.

Our services are designed to meet the unique physical, psychological, social and spiritual needs of our patients and their families, with the aim of enabling people to achieve the best possible quality of life.

All services are provided free of charge to those who need them.

These services fall into three categories:

• Nursing and Medical Care
• Patient and Family Support
• Therapies

Referrals

Most people are referred to us via a medical professional. If you feel that you might benefit from our services, it is useful to discuss this with your GP, hospital staff, or Community District Nurse.

We do accept self-referrals but would need to liaise closely with your medical adviser to ensure that you receive the most appropriate care. Please fill in self-referral form at the bottom of this page and fax it back to us on 01622 718920.
